2/10/2024 EnchantaVaughn Approved QMHPA greeted Chevelle upon arrival to the home setting. Upon arrival to the home setting, Chevelle appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her evening. She indicated to QMHPA her evening was going well. Chevelle did not present any symptoms of S/I or H/I at the time of the session. QMHPA used a person-centered approach to assist Chevelle with improving family relations and independent living skills. QMHPA engaged Chevelle in role play and role reversal activities to educate her on appropriate, assertive, and respectful ways to express her feelings and concerns towards her family. QMHPA encouraged Chevelle to discuss and verbalize her frustrations and feelings of depression. QMHPA engaged Chevelle in calming techniques (slow breathing) to assist her with decreasing anger and frustration. Chevelle maintained a cooperative attitude throughout the session; however, she displayed anger when expressing her feelings by using profanity and swearing. Chevelle actively engaged in the role play and role reversal activities and demonstrated a clear understanding of appropriate ways to communicate her thoughts and feelings to her family.
